HTTP Negotiate can create and send a Kerberos token before receiving a
server challenge. The SSPI backend only supplied Schannel endpoint
bindings when a challenge token was present, causing IIS with EPA
enabled to reject preemptive Kerberos authentication with
STATUS_BAD_BINDINGS.
Build the SSPI input descriptor independently of the challenge token so
Schannel endpoint bindings are included on both initial and subsequent
InitializeSecurityContext calls.

- Switch the Windows SSPI identity struct to SEC_WINNT_AUTH_IDENTITY_EX
to use !ntlm in PackageList to prevent NTLM from being offered.
- For GSS filter out NTLMSSP OID, and restrict via gss_set_neg_mechs()
to prevent NTLM from being offered.
- Extend the GSS-API debug stub layer to support the NTLM blocking logic
without a real Kerberos environment.
- Update test 2057 to check that negotiate auth is silently skipped with
no Authorization header when only NTLM stub credentials are available.
- Add SPNEGO NTLM blocking test 2093 which verifies that Kerberos
credentials still succeed when NTLM is blocked within SPNEGO.
- Suppress tests valgrind leak for MIT krb5 gss_display_status, since
the leak is in the library and not in curl.
To suppress the tests valgrind leak, the wildcard '...' bridges over an
anonymous frame inside libgssapi_krb5.so that valgrind reports as '???'.

Add support for the native GSS Framework on Apple targets, which is
Apple's fork of Heimdal. This option allows to drop Apple's deprecated
MIT Kerberos compatibility shim "mit-krb5/1.7-prerelease".
Source code uses different headers, other than that no source changes
are necessary.
You can enable by configuring with:
autotools: `--enable-gssapi-apple`
CMake: `-DCURL_USE_GSSAPI=ON -DCURL_GSS_FLAVOR=Apple`
These options are experimental, and may receive breaking updates till
the GSS backend selection logic settles, for Apple and also for the GNU
and MIT implementation.

DecryptMessage() decrypts the buffer in place, overwriting the original
contents. It does not allocate any new buffer so the single original
buffer should be freed using the same memory "system" that allocated it.

Add `data->state.origin` as the origin the transfer is sending the
current request to/gets the response from. Use it for request specific
properties like authentication, hsts and cookie handling, etc.
Unless talking to a forwarding HTTP proxy (e.g. not tunneling),
`data->state.origin` and `conn->origin` are the same.
With a forwarding HTTP proxy in play, `conn->origin` is set to
`conn->http_proxy.peer` and `conn->bits.origin_is_proxy` (a new bit) is
set.
Remove the connection bits, now replaced with:
* `conn->bits.socksproxy` -> `conn->socks_proy.peer`
* `conn->bits.httpproxy` -> `conn->http_proy.peer`
* `conn->bits.proxy` -> `(conn->socks_proy.peer || conn->http_proy.peer`)
* `conn->bits.tunnel_proxy` -> (`conn->http_proy.peer && !conn->bits.origin_is_proxy`)
* `(conn->bits.httpproxy && !conn->bits.tunnel_proxy)` -> `conn->bits.origin_is_proxy`
Rename `noproxy.[ch]` to `proxy.[ch]`. Move the connection proxy setup
code from `url.c` to `proxy.c`.
Remove `data->info.conn_remote_port` as no one uses it.

CURLOPT_GSSAPI_DELEGATION exposes distinct modes:
CURLGSSAPI_DELEGATION_POLICY_FLAG is documented as delegating only when
OK-AS-DELEGATE policy permits it, while CURLGSSAPI_DELEGATION_FLAG is
unconditional. The new SSPI implementation checks for either bit and
sets ISC_REQ_DELEGATE, so a caller requesting policy-limited delegation
is put on the same SSPI path as unconditional delegation.
In addition, curl's existing protection that avoids reusing a connection
when the GSS delegation setting differs was guarded only by HAVE_GSSAPI;
SSPI-only builds now have an effective delegation option, but the
connection's delegation setting was neither copied nor compared. This
would cause Windows SSPI Negotiate/Kerberos authentication to delegate
credentials contrary to the caller's selected policy or reuse an
already-delegated authenticated connection for a transfer that requested
no delegation.

The SASL service name, used in authentication, is part of curl's credentials
when authenticating to a server/proxy. Make it part of `struct Curl_creds`.
Change code to use `creds` to obtain a service name. By tying creds used
to the connection, connection reuse is also only allowed when the service
name matches.

Authorizdation credentials are kept in `struct Curl_creds`. This contains:
* `user`: the username, maybe the empty string
* `passwd`: the password, maybe the empty string
* `sasl_authzid`: the SASL authz value, maybe the empty string
* `oauth_bearer`: the OAUTH bearer token, maybe the empty string
* `source`: where the credentials from from
* `refcount`: a reference counter to link/unkink creds
A `creds` with all values empty is equivalent to NULL, e.g. no `creds`
instance. With reference counting, `creds` can be linked/unlinked
in several places.

(instead of a native GSS-API implementation), so Kerberos delegation can
be requested during SPNEGO/Negotiate authentication.

`struct Curl_peer` keeps information about a communication endpoint
together. It will replace `conn->host` and `conn->conn_to_host` and
proxyinfo host. It will also become part of `struct ssl_peer`.
It has a reference counter, so an instance can be shared between
connections and filters.
Elminiates `conn->host` and `conn->connect_to_host`, used in the
proxyinfo structures. Passed to DNS resolution and socks filters, etc.
Pass peer to http proxy and socks tunnel filters. Use peer in dns filter
and resolving. Make `Curl_peer` a member in the `struct ssl_peer`.
Add `docs/internals/PEERS.md` for documentation.

Extend `#pragma diagnostic push`/`pop` guards to the whole codebase
(from tests and examples only) to disable it for GCC <4.6. Rename guard
to `CURL_HAVE_DIAG` and make it include llvm/clang to be interchangeable
with `__GNUC__ || __clang__` in this context.
The above means no longer disabling certain warnings locally, so pair
this with disabling all picky warnings for GCC <4.6.
Also:
- drop global workarounds for misbehaving GCC <4.6 compiler warnings.
Not needed with picky warnings disabled.

Backtrack on previous change that aimed to solve the wrong `share.h`
being included. It turns out it did not fix this issue. At the same time
it introduced relative header filenames and the need to include the same
headers differently depending on the source files' location, reducing
readability and editability.
Replace this method by re-adding curl's lib source directory to the
header path and addressing headers by the their full, relative name to
that base directory. Aligning with this method already used in src and
tests.
With these advantages:
- makes includes easier to read, recognize, grep, sort, write, and copy
between sources,
- syncs the way these headers are included across curl components,
- avoids the ambiguity between system `schannel.h`, `rustls.h` vs.
local headers using the same names in `lib/vtls`,
- silences clang-tidy `readability-duplicate-include` checker, which
detects the above issue,

Slight downside: it's not enforced.
If there happens to be a collision between a local `lib/*.h` header and
a system one, the solution is to rename (possibly with its `.c`
counterpart) into the `curl_` namespace. This is also the method used by
curl in the past.

- toolx/tool_time: apply this to an include, and update VS project
files accordingly. Also dropping unnecessary lib/curlx header path.

When the compiler supports C99.
- map logging functions to macro stubs when verbose logging is disabled
and the compiler is C99. Make sure these stubs silence unused variable
warnings for non-variadic arguments.
Before this patch they mapped to function stubs, the same codepath
used for C89 compiler in this configuration.
- introduce new macros to tell the compiler which code to include
when verbose code is active, or inactive:
- `CURLVERBOSE`: defined when verbose code is active.
To enclose blocks of code only used for verbose logging.
- `VERBOSE(statement);`:
compile statement when verbose code is active.
To mark code lines only used for verbose logging.
- `NOVERBOSE(statement);`:
compile statement when verbose code is inactive.
To suppress warnings for arguments passed to logging functions via
printf masks, e.g. `NOVERBOSE((void)ipaddress);`, yet keeping
the warning in verbose builds.
Note these macros are not the same as `CURL_DISABLE_VERBOSE_STRINGS`.
Verbose code is always active in C89 mode (without variadic macro
support).
- drop existing uses of `CURL_DISABLE_VERBOSE_STRINGS` where redundant,
or replace with the above macros. Ending up reducing the number of
`#ifdef`s, and also the number of lines.

This allows builds know about all schemes - but only have the protocol
implementations for those actually built-in.
It further allows multiple protocols to reuse the same protocol setup
and functions for both TLS and non-TLS implementations instead of
needing two (or more) structs.
The scheme information is now in 'struct Curl_scheme' and all the
function pointers for each scheme/protocol implementation are in struct
Curl_protocol.
The URL API now always work with all known protocols.
